Payment options:

• Cash or Check: Gladly accepted for all services.
• Credit Card: We accept Visa, MasterCard, Discover, and American Express.
• CareCredit: Flexible financing available—apply at www.carecredit.com or in our office.

Please note: Payment is expected at the time services are rendered.

As part of our service, we’ll submit claims to your insurance provider and monitor their progress. Please keep us updated on any changes to your insurance coverage. Ultimately, you are responsible for the total cost of care provided by our office, regardless of insurance reimbursement. Most insurers respond within four to six weeks. Any remaining balance will be billed directly to you.
